I think a simple off camera retraction of a long hose would be quite easy. Or an apparatus inside the hand of the male actor. like a small squeeze bottle with a small applicator or hose attached. There are many different ways they could achieve this and they are very clever with it and ingenious.

I will ask my contact the number of ways they achieve this and get back to you! But in my mind it is a concrete a certified staged scenario in this instance. My conviction of that is even higher now based on the images you provided.

Any names for which actress is the best at getting throat fucked upside down? For example; regularly does that position , regularly takes it hard and long in that position etc. that’s my favourite position/moment of the film as that’s when it usually ramps up, or peaks in the male actor being more ‘enthusiastic’ getting to go primal
