She probably retired without making an announcement. Her last release was sometime last year.

100%. It can be misunderstanding or just lazy reading, but once it's out there others will start to copy it and leads to confusion. Humans are bad enough, but add automated data collection and it's out of control.

A very annoying one for me is Kato Shiori:
