I’ve come to a conclusion, the stars have to align. I think it is a combination of the actress and actor’s physiology, the angle, the intensity, I think the degree to which the actress tenses up or relax her muscles as well, friction and the weather outside. Though the biggest factor should be the actress’ physiology as you already observed, the same dude with the same set up produces different results. I will note too that even within the same scene, I’ve seen sometimes that there’s no bulge for most of the time, but there will be for short periods, so some magic ingredient needs to be there.
Before I go on about this topic, I must recommend MISM-246, the second actress at 02:02, probably the most pronounced bulge I can remember.

While some actress are evidently more prone to bulging, that’s also not a guarantee. Take Mio Ichijo, one of my fav, for instance, she had one of the most incredible scene in ENKI-028 (~ 02:41) (also fully clothed - that’s another topic for another day). I’ve waited till this day to see her bulges like that but it just doesn’t come by, not even in ANAV-001. I don’t know if there’s a clear cut answer, even if just my own opinion. I feel the aesthetic (naked, fully clothed, anywhere in between) just have to synergise with the scene and elevate my viewing exp.
There’s something about Mio Ichijo in that ENKI-028 maid outfit, or the excellent lady’s, well every scenes but especially the first in MISM-344, that just work so well. Another good example is MISM-204 (which follow a specific format that I really like, it’s like a mini series with MISM-222, 216, 252 and recently CKCK-011!?), the second scene typically features a sheer top and some very intimate no-nonsense throating.
There’s also I think a feeling out there that the fact they are not even taking off her clothes in say, the DFE titles, is showing that it’s all about doing her mouth. But the other end can claim that fully naked = more bare and vulnerable, and visually better for some.

ENKI-094 has me VERY impressed with Ichika / Ichihana's throat work. Particularly the throatfuck w/ throatpie which takes-place on the bathroom floor (timestamp 1:08:55). She took it at full depth, and held her breath for a total of 47 seconds! Looking forward to seeing Ichika / Ichihana in future throat titles, if any — she clearly has great potential.

Can't really offer the same praise to HUBLK-065, though, despite both actresses demonstrating superb throat talent — the issues are directorial in nature.
The 1st scene with Momose disappoints, as it felt like throatfucking was far from the focus. It plays more like a conventional rough porno, with a dash of vomit fetish, and *some* added throat work. No throatpie either — the guy merely pulls out and gives her a facial.
Same complaint for the 2nd scene.

If you want to see Sakuma at her best, my recommendation would be MIAB-429 instead.
Starting around the 1h 53m mark, she take two consecutive throatpies in this position. And the diagonal camera angle (per the screengrab below) does a top-notch job of showcasing her neck pulsations, as well as those lovely petite tits.
