Rare, and probably in violation of contract. Kaho Shibuya sued the producers of 2 of her videos that were released uncensored. She sued for 7 million yen. I never heard how the lawsuit came out; presumably it settled without a trial (not sure about Japan's legal system). The very fine website Javdesu.com has separate categories for 'uncensored leaked' and 'mosaic removed'. There's no mistaking one for the other.
