Took some searching as no names are listed for the movie but the code is SDDE-116. The writing in the actual video has her name as 越山サキエ Sakie Koshiyama, but that may just be the name of the character she plays.

you're right, Sakie Koshiyama is just her role name in that video. i have tried to search for that name before, but ended up with nothing.
really glad that finally someone is able to ID this movie
