Took a bit of searching as this is only the second half of the movie, but luckily the end title told me it was a Madonna movie directed by きとろね川口, which narrowed it down a lot.
Turns out the movie is JUC-963. There are several threads here with torrent and direct download links, but I don't know whether they still work. Just do a forum search on JUC-963 and try them.
