If a DVD is known it can simply be asked for as a request. However when it's unknown it's odd to request it when we don't know what it is. Perhaps someone might know but the point of requests is to provide as much info as possible in hopes to get what we want right? The proper Japanese title, the DVD code, the link to this DVD and a cover image is all that is needed. You can add more about "how much you want it" and "how long you've been looking for it" but those usually don't help at all.
