Can't tell you the name of the actress, but the movie is RCT-858.
Sougouwiki lists 2 of the 3 actresses: 高嶋ゆいか／倉多まお. Unfortunately neither of those names line up with the stage names used in the video. The woman you want does have one of the two last name characters in common, the 倉, so maybe 倉多まお Mao Kurata is the one?
