It seems her name is Karen Kotaki - 小滝かれん (小滝香蓮) and is (or was) a gravure idol.
The video is from a pink film, i didnt found the code but the title is "その男、エロにつき　アデュ～！久保新二伝". A couple of classic jav actresses appear in that vid.
