In fact, MISM-389 and MISM-396 clearly have different content. However, if you look at the videos that are widely circulating online right now, both of them appear to have the same content. If you check the descriptions for each title on Fanza, the video that is currently circulating is actually MISM-396.
Why it ended up being distributed as MISM-389, which was released earlier, is unclear. One possibility is that incorrect data was sold in the early stage of the release, and someone in China ripped it and spread it widely online. At least, if you purchase MISM-389 legitimately from Fanza now, you should be able to watch the correct version.

The weirdest part is that the wrong version of MISM-389 actually has the title screen of the real MISM-389:

Yes, most likely the studio made a mistake at the time of the initial release and mixed up the titles. These two titles have similar content, and although I haven’t confirmed it, they were probably filmed around the same time as well. That mistaken version was probably released as it was, and then it spread online through Chinese pirate copies.
