Complete decensored is a subjective term.
There are true "uncensored" leaks of regular JAV where the original footage, before the pixelation was applied , has been stolen and released. I think there is a thread on this site somewhere about that.
Decensored JAV is impossible as the pixelation process removes the original parts of the video that feature the genitals and replaces it with a pixelated image.
There are some tools, JavPlayer being one of the more famous ones that attempt to "reverse" this process. In fact what they do is remove the pixelisation and replace it with an AI estimate of what they think should be there. The AI uses a model or set of model files to reconstruct the missing parts. results vary in quality with some unwatchable and others quite good. There should be a discussion on here somewhere, or you could try https://scanlover.com which has a huge thread on the subject.
For downloadable movies where JavPlayer has been used, look in https://sukebei.nyaa.si Search for "decensored" or mosaic removed.

Blurry guesstimate genitals aren't much better than a mosaic in my mind so I never bother with them, especially since there's presumably reencoding done to the video afterwards that further degrades the quality of the source.

The quality of those "decensored" movies varies a lot. It goes from those very blurred ones you recognise right azway to some where you can barely see they were modified. The last category doesn't have those annoying watermarks either, but they are a very small minority of what's out there so it's pretty hard to find them. In most cases I feel you're better off with the censored version.
Have you got an example code and location for one? I'd legitimately like to see de-censoring done right.
Here’s another good example. Older mosaics can turn out better because the mosaic size was smaller or it used a simpler algorithm for the mosaic generation.
