According to the cover, she's only identified as Chii (ちぃ). I checked a few AV DVD websites and all of her movies (I found 4) only show her name as ちぃ.

I bought one about a year ago, and it does really feel pretty damn good if you warm it up in hot water first. Even if you don't warm it up, it still gets the job done. Clean up is kind of a pain in the ass, but it's quick; should take you no more than a minute or so. They also last a long time...
