The first two pictures are from older model kits. That means you only get the parts and have to do the assembling and painting yourself. Those kits are sold in small numbers and are very hard to get. Sometimes you can find figures like that on the japanese yahoo auction site. Here is an example...

That's a good question ;) Most of the more revealing models are made by independent company’s in very small numbers, often sold at special events in Japan only. Very often these are model kits, so you don't get the complete figure just the pieces that you have to assemble and paint yourself...
