around the 25 minute mark the IV just closes down or experiences issues, you also can't skip forward past that point either, even on the scene selection clicking on a scene past the second one causes it to freeze and break down. Seems basically that he rest of the DVD wasn't burned in it's entirety.

i'd have to guess Yuuri Himeno, atleast that was her name when she still did gravure, not sure what her AV name is.
