How long does it take to no longer need mod approval for comments?

I've been on this site for a couple months, but whenever I try to reply to a thread I need to 'wait for moderator approval' and my comment gets auto-deleted a few hours later. Anything I'm doing wrong?

if there is a problem there is always IRC, not always there 24/7, but some one might be able to help, or just hung about for a while
