It depends on the context... the original meaning is more about precocious preteens and the onset of puberty, generally 11-13 years old (Dolores was actually 12 in the book) and not including teens in general or little girls. In the context of lolicon and hentai, it's more any age before puberty. Many people use it to mean teens in general but I find that a weird usage as if Dolores had been 16 years old the book wouldn't have been very controversial or have caught on so much. But the meaning has also evolved to mean young looking teens and even adults. Basically there is no general definition that can be applied to all contexts, but generally loli porn = young looking, lolicon = any age before puberty and loli girls = preteens / early teens.
